• Exam - airway , lung , heart

• Guedel airway - ช่วยให้ tongue forward - ใช้ตอนเตรียมคนไข้ตอน intubation

Guedel airway

• LMA - Laryngeal mask airway - ใช้ maintain ระหว่าง Sx กรณีที่ spontaneous ventilation

• หรือใช้กรณี difficult airway

Laryngeal Mask Airway

• ยังมีตัวช่วยอีก 2 อันคือ

• Stylet size 14, 10 , 6

• Bougie

Stylet , Bougie

• เครื่องมือที่ทำให้เห็นชัดขึ้น

• Laryngoscope

• Glidescope

• Storz Videoscope

• Fiberoptic bronchoscope

• Airway assessment ใช้จาก - Mallampati Score <- ใช้ง่าย

• ให้ผปนั่ง 30-45*

• อ้าปากกว้างที่สุด

• ดูว่าเห็นอะไรบ้าง ถ้า MP 1 เห็นตั้งแต่ฟัน ถึง uvula

• แต่ถ้า MP IV เห็นแต่ tongue

• ง่าย แต่ไม่ specific กับ difficult airway

Mallampati Score

• Monitor ใน US จะเพิ่มอีก 2 อัน คือ end-tidal CO2

• temp เข้าไปในปากหรือ nose

• Fentanyl 1-3 micrograms/kg

• Propofol 2-2.5 mg/kg

• Rocuronium 0.6 mg/kg

• ถ้าใช้ neuromuscular block - ควรจะมี monitor n stimulator ด้วย

Monitor ด้วย n stimulator

• Securing Airway

• mask ต้อง tight - nose ,month

• chin elevated
