254 Cardiomyopathy and Myocarditis

Myocarditis

• inflammatory of heart

• multiple causes - แต่ most common-> infective , common จะเป็น viruses / protozoan -> Trypanosoma cruzi

• direct invasion

• cardiotoxic substance

• Ch inflammation

• ss : decreased systolic function

• severe infection -> cytokine release -> depress cardiac function transiently

Infective Myocarditis

• viral เข้าทาง respiratory / GI

• specific receptors such as coxsackie-adenovirus ที่ heart

• 1st host response -> innate immune response

• 2nd acquire immune response - T-cell , Ab

• cytokine release -> ทำลาย disrupt collagen ทำให้ -> ventricular dilation

• pro-fibrotic factors -> interstitial fibrosis

Clinical Presentation of viral Myocarditis

• Heart failure , มี chest pain <- pericarditis หรือ ac myocardial infarction

• atrial / ventricular tachyarrhythmias

• หรือ pulmonary / system emboli จาก intracardiac thrombi

• EKG หรือ echo - อาจจะพบ abn ได้

• young - middle age ที่มี progressive dyspnea , weakness ไม่กี่วันหลัง viral syndrome , fever , myalgias

• มีไม่มากที่จะ fulminant myocarditis ไม่กี่ชม. หลัง severe febrile resp to cardiogenic shock

• multiorgan failure
