Epicanthoplasty (1)

• Surgical technique มีได้ทั้งแบบ Z , VY , W plasty

• ต้องดูก่อนว่าจะ Mx อะไรบ้าง

1. How will Mx skin redundancy and deficiency ?

2. How will control extent of caruncle exposure ?

3. Can transverse incision placed ใน nonvisible area , หรือ along relaxed skin tension line ?

4. possible หรือไม่ที่จะ leave skin bridge ระหว่าง epicanthoplasty incision และ double eyelid incision

Anatomy Epicanthal fold (2)

• cause - multifocal

• Z-shape kinking of orbicularis muscle fiber

• excess underlying orbicularis muscle and fibroadipose tissue

• no or little attachment medial levator aponeurosis

• epicanthal fold มีทิศทางเดียวกับ fiber preseptal orbicularis muscle

postmortem eyelid show medial canthal tendon และ orbicularis oculi muscle
